The Utah Food Industry Association Insurance Trust was started 50 years ago to serve the group insurance needs of members and their employees. The Trust is currently comprised of Association members and their full-time employees, together forming a solid, reliable insurance program which is underwritten and fully insured by Regence Blue Cross Blue Shield of Utah (BCBS).

There are five health plans from which to choose with additional options as described here. All plans require enrollment in the Term Life Insurance Plan.

Utah Food Industry Association Insurance Trust

This information gives brief summaries of the programs available, but is not intended to give total descriptions of benefits or exclusions.

Eligibility

The Utah Food Industry Association Insurance Trust is open to any member of the Association and their full-time (an average of 30 hours per week) employees.

For employers with ten (10) or more eligible employees, 75 percent of the eligible employees must participate on the plan and be approved by BCBS underwriters. The effective date for these employees will be on the first day of the month following at least one month of full-time employment, per employer's policy. Each participant is required to submit a health statement.

For employers of less than ten eligible employees, 100 percent of those eligible must participate and all employees must furnish a health statement form, and be approved by BCBS underwriters. When approved, employees are effective the first day of the month following the date of approval.

Dependents eligible for coverage under these plans are an employee's spouse and an employee's unmarried children under 26 years of age and are at least 50 percent dependent upon the subscriber for maintenance and support.

ValueCare and HMO Blue Plans
T
he Utah Food Industry Association Insurance Trust offers ValueCare, a preferred provider organization (PPO), and HMO Blue, a health maintenance organization; both underwritten by BlueCross BlueShield of Utah.

Conventional Insurance Plans
A variety of conventional insurance plans are offered by the Trust. The table below shows differences between the plans.

The deductible on each plan is per individual per calendar year. Family maximum is met when three members each meet their individual deductible during the same calendar year.

Out-of-pocket maximum is the maximum amount an individual will be required to pay per calender year. Out-of-pocket maximum applies to full family when three individual maximums are satisfied.

Maternity
Maternity covered as any other illness. Newborn Care at the time of delivery is considered as a covered charge as long the newborn is a covered dependent.

Pre-Authorization
Pre-authorization of select surgeries is required:
bunionectomy, cataract removal, carpal tunnel surgery, cholecystectomy, dilation and curettage (D&C), hemorrhoidectomy, herniorrhaphy, hysterectomy, knee surgery (including excision of knee cartilage), laminectomy, ligation and stripping of varicose veins, mastectomy and other breast surgery, prostatectomy, submucous resection, tonsillectomy and/or adenoidectomy.

Employee Assistance

Provides short-term confidential assistance by phone for personal problems that may be interfering with your life or your work such as stress, parenting, depression and marital issues. Employee Assistance services are easily accessible, confidential and available 24 hours a day at no out-of-pocket cost. For an appointment, please call during regular business hours, Monday through Friday: In the Salt Lake area call 261-4006. In the Ogden, Provo, and St. George areas call: (800) 218-6333. All other locations call (800) 833-3031. Crisis services are available 24 hours a day, seven days a week at (800) 833-3031.

Special Beginnings
Provides supportive services to all pregnant mothers, and to help identify, educate and monitor women who may exhibit high-risk symptoms during pregnancy. Not only is this program free, but we'll send you an illustrated prenatal and child care book, helpful literature and a gift just for participating. If you are pregnant call (888) 569-2229 and enroll in the program. It's a program designed with you and your baby's health in mind. We also provide a toll-free 24-hour phone line staffed by nurses who have experience working with pregnant mothers and newborns.

Life Insurance

Term Life Insurance Plan
All persons enrolled on the Association plans are required to be enrolled on the Group Term Life plan.

Life Insurance for Employees

Life and Accidental Death and Dismemberment (AD&D) amounts decrease to 50% at age 70 and terminates at retirement.

Optional Life Insurance
Participants may purchase additional term life and AD&D Insurance (including waiver of premium) in $10,000 increments, up to a maximum of $1,000,000 total insurance protection. This coverage can be purchased on an individual basis, in addition to the $10,000 or $15,000 amount provided under the group plan.

Optional Short Term Disability
Benefit Begins
On the 15th day following an accident, or on the 15th day of a continuing illness.

Benefit Duration
The end of disability or a maximum of 26 weeks.

Weekly Benefit Amount: $250

An individual must be totally disabled and under the care of a physician. Coverage is nonoccupational only and terminates at age 70. It does NOT replace Worker's Compensation.

Optional Prescription Drug Plan
The Optional Drug Plan is available to groups of 10 or more employees. It provides direct reimbursement of allowable prescription drug charges. The plan provides payment of prescribed drugs in excess of $10.00 (generic) or $15.00 (name brand) or 20 percent, whichever is greater, per prescription.

Optional Vision Program
Includes one routine vision exam per calendar year. Other benefits include a portion of either one pair of glasses or one pair of contacts in a two-year period. The standard Vision Plan is outlined as follows:

  • Routine Exam: $30.00
  • Frames: $40.00
  • Single Lens: $40.00
  • Bifocal Lens: $60.00
  • Trifocal Lens: $75.00
  • Contact Lens: $100.00

Each Member is covered for one "routine" vision exam per calendar year. Other benefits are provided at one pair every two years; that is, either one pair of glasses or one pair of contacts in a two-year period.
