The Trust is currently comprised of Association members and their full-time employees, together forming a solid, reliable insurance program which is underwritten and fully insured by Regence Blue Cross Blue Shield of Utah (BCBS). There are five health plans from which to choose with additional options as described here. All plans require enrollment in the Term Life Insurance Plan. Utah Food Industry Association Insurance Trust This information gives brief summaries of the programs available, but is not intended to give total descriptions of benefits or exclusions. The Utah Food Industry Association Insurance Trust is open to any member of the Association and their full-time (an average of 30 hours per week) employees. For employers with ten (10) or more eligible employees, 75 percent of the eligible employees must participate on the plan and be approved by BCBS underwriters. The effective date for these employees will be on the first day of the month following at least one month of full-time employment, per employer's policy. Each participant is required to submit a health statement. For employers of less than ten eligible employees, 100 percent of those eligible must participate and all employees must furnish a health statement form, and be approved by BCBS underwriters. When approved, employees are effective the first day of the month following the date of approval. Dependents eligible for coverage under these plans are an employee's spouse and an employee's unmarried children under 26 years of age and are at least 50 percent dependent upon the subscriber for maintenance and support. The Utah Food Industry Association Insurance Trust offers ValueCare, a preferred provider organization (PPO), and HMO Blue, a health maintenance organization; both underwritten by BlueCross BlueShield of Utah.
A variety of conventional insurance plans are offered by the Trust. The table below shows differences between the plans.
The deductible on each plan is per individual per calendar year. Family maximum is met when three members each meet their individual deductible during the same calendar year. Out-of-pocket maximum is the maximum amount an individual will be required to pay per calender year. Out-of-pocket maximum applies to full family when three individual maximums are satisfied. Optional Life Insurance Optional Short Term Disability On the 15th day following an accident, or on the 15th day of a continuing illness. Benefit Duration Weekly Benefit Amount: $250 An individual must be totally disabled and under the care of a physician. Coverage is nonoccupational only and terminates at age 70. Optional Vision Program
